		New Account Conversion Checklist: to begin the process of closing your accounts at other financial institutions
		Direct Deposit Change Request Form: notify your employer (or other source) to begin sending direct deposits to your new 1st CCU accounts. Please allow 2-3 weeks for your Direct Deposit changes to take place.
		Automatic Withdrawal Change Request Form: notify current automatic payment vendors (utilities, mortgage, insurance, etc.) that future payments should be deducted from your new 1st CCU account. Please allow 2-3 weeks for your Automatic Withdrawal changes to take place.
		Account Closeout Authorization Form: to complete the process of closing old account(s) once all transactions have cleared
